Important: attach your supporting documents and include: First Name, Last Name, Member Number, email, phone number, and nature of the request (FSI medical care/physical therapy, ambulance, birth allowance, death benefit). To ensure fairness among all members, FSI claims (medical care, physical therapy, ambulance) are funded through an annual budget: submit your expenses from the previous year between January 1 and March 31 of the following year. We then consolidate all claims to ensure equitable distribution and proceed with the payments. In recent years, 100% of valid FSI claims have been reimbursed. Birth and Death Benefits can be requested at any time.
